Clinical Pharmacist Services are needed at the Naval Hospital 29 Palms,CA in support of the Medical Home Port initiative. Services may also be provided at associated Area Branch Health Clinics within a 35-mile radius of the hospital.
The Medical Home Port program is a new model of patient and family-centered health care delivery for primary care that is team based, comprehensive, and designed to fully meet the complete primary care health and wellness needs of patients.
License & Certification Requirements:
- Must possess a Basic Life Support Card (BLS) by the American Heart Association.
- Must possess and maintain a current, valid, unrestricted license to practice Pharmacy in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
- Must possess a Doctorate degree in Pharmacy (PharmD) from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE).
Must possess a Doctor of Pharmacy (PharmD) degree from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) and have demonstrated competency in medication therapy management. Competency in medication therapy management may be demonstrated by possessing at least one of the following:
Successful completion of a residency program approved by the American Society of Health-System Pharmacists (ASHP) or the American College of Clinical Pharmacy (ACCP).
Specialty Board Certification in either Pharmacotherapy or Ambulatory Care through the Board of Pharmaceutical Specialties (BPS).
A minimum of 3 years of documented Pharmacist experience within the last 5 years in the clinical ambulatory care setting.
