Shift: 8-hour, Days
Under the direction of the VP of Care Management, the Ambulatory Clinical Pharmacist is an integral part of the St. Joseph Heritage team, and works closely with the Ambulatory Clinics, Care Management programs such as CARE Connect and Discharge Follow-Up, Transitions of Care, Nurse Advice Line, Transitional Medical Clinics, along with other clinical integration activities. In collaboration with the primary care physician, the Clinical Pharmacist performs pharmaceutical patient assessment (initial and reassessments), develops and manages the patient's chronic illness pharmaceutical care plan, and coordinates that plan with the health care team.
+ Performs pharmaceutical assessment and reassessment of patient medication regimen including reconciliation and adherence to patients in the ambulatory setting.
+ Acts as a consultative resource to physicians and other members of the health care team to assess patient's medications for interactions, duplications, contraindications, potential formulary coverage issues, and cost effective alternatives, and makes recommendations to the prescribing provider.
+ Coordinates the pharmaceutical care of patients including the initial pharmaceutical assessment and reassessments of ambulatory patients.
+ Develops appropriate care plans that are tailored to the patient's needs. Upon request, executes these care plans through comprehensive patient monitoring and documents changes in the patient's therapy or condition in the medical record.
+ Analyzes lab results and other clinical data and provides appropriate clinical monitoring. Orders appropriate laboratory tests necessary for monitoring outcomes of medication therapy according to established protocols or in consultation with the primary care physician.
+ Communicates relevant issues to physicians and care team members.
+ Assists in the teaching of health care providers, patients and their family members/caregivers about the patient's medications, directions for use, and other information as deemed necessary. Identifies barriers to patient compliance and assists patient in overcoming barriers.
+ Proactively identifies performance improvement opportunities through data analysis and discussion with subject matter experts to achieve pharmaceutical-related total cost of care and quality results.
+ Coordinates with clinical departments, clinical operations, health plans, and relevant stakeholders to drive improvement around identified pharmaceutical-related cost reduction and quality improvement areas and delivers and tracks needed results.
+ Develops and implements prescribing protocols in collaboration with members of the health care team.
+ Basic computer skills in a Windows operating environment including Microsoft Word, Excel, and an e-mail system.

+ Knowledge of pharmaceutical chemistry, materia-medica, and pharmaceutical compounding methods preferred.
+ Knowledge of clinical pharmacy practice to include: patient monitoring, interpretation of drug serum levels, literature evaluation, formulary enforcement, drug utilization review, and drug interactions preferred.
+ Proficient in all MS Office applications and Excel preferred.
+ Bilingual English/Spanish communication skills preferred.
**Minimum Position Requirements:**
+ Graduate from an accredited school of pharmacy.
**Experience** : 3 years experience with direct patient contact in acute or ambulatory care setting.
+ Requires a valid California Pharmacist license.
+ AHA BLS
**Preferred Position Qualifications:**
**Education** : Coursework/Training - Residency in ambulatory or managed care programs.
+ Chronic disease state knowledge base and related pharmaceutical counseling and/or motivational interviewing experience.
+ EHR experience (interpreting and inputting data).
